A risk assessment is often requested by courts, child protection agencies or other institutions wanting to understand the risk an individual poses to engage in a particular behaviour. Documentation required from the referral source or client (when available or relevant): official criminal record, police records and/or official documentation of prior charges/convictions, correctional records, previous psychological/psychiatric assessments, documentation from CAS, school transcript, and a letter of employment (from current employer, detailing length and type of employment).
